I. Paper-based Unit Evaluations
Printing process
Download the questionnaire from:
http://www.adm.monash.edu.au/cheq/evaluations/unit-evaluations/fac-unit-eval-quest-2008.html
- Click on the
survey link to open.
- Select the print
option ensuring you print the survey in DUPLEX mode with PAGE SCALING
set to "NONE". This is very
important as the surveys will not scan if printed over two pages.
- Select the number
of copies you require.
- Print.
- Place surveys in
an unsealed envelope and write Unit Details in the space provided eg. VCM4002_S1-01_CAULFIELD_DAY (these details are printed at the top of the
survey form).
Surveys are administered between week 9 and week 14.
Please inform the CHEQ Evaluations Unit of the number of return envelopes you
require.
Important
- Where completed paper-based unit evaluations are returned to CHEQ on
forms that are of poor print quality, a manual transcription of the data
is required.
- CHEQ will notify immediately the relevant Associate Dean (Teaching)
by email advising that the issue has arisen and that a charge (at cost)
will be incurred.
Distribution and administration process
- Independent individual administers the survey.
- The administrator opens the sealed envelope containing the questionnaires
in the classroom and distributes the questionnaires to the students.
- Whilst students fill in the questionnaire, the administrator
fills in their details (name, phone number etc) on the self-addressed
return envelope. This is an essential step as random audits are
carried out to ensure that the proper process has been undertaken.
- Put the completed questionnaires in the self-addressed return
envelope.
- Seal the envelope in front of the class.
- Sign across the sealed flap.
- Put sealed self-addressed return envelope in internal mail.
- Return any blank questionnaires in a separate envelope to CHEQ
Evaluations Unit.
Important
- Administrator must sign across seal in the space provided. If
the seal on the return envelope is broken, then the entire unit may not
be processed by CHEQ.
- Do not alter any information on the questionnaires as such changes are
not recognised by the software.
Points to highlight to students when handing out questionnaires:
- They are totally anonymous.
- Data is collated by an independent body, The Centre for Higher
Education Quality (CHEQ).
- Results will be available on the CHEQ website: http://www.adm.monash.edu.au/cheq/evaluations/Unit_Evaluations/unit-eval-reports.html
- If the seal on the return envelope is broken, then the entire unit may
not be processed by CHEQ.
- Students should read instructions on the questionnaire carefully.
- Students can write comments on the survey forms and these will be
processed unedited.
II. Web-based Unit Evaluations
- Evaluations Unit activates online questionnaires.
- Evaluations Unit triggers system to send an email to students in each
unit advising them of the URL for the questionnaire.
- Students complete and submit online surveys.
- Reminder emails to students who have not completed the survey are
triggered in the system (weeks 11 and 13).
- Access to Unit Evaluation questionnaires are closed before
examinations begin (Week 14).
Note: Students will not be able to submit multiple surveys
