Centrelink consent and authorisation

You can authorise Monash to obtain your details directly from Centrelink via electronic means using Centrelink Confirmation eServicesby agreeing to the consent below. This consent will be used for the sole purpose of authorising Centrelink to provide information to Monash University to assess your eligibility in relation to scholarships and bursaries offered by Monash University.

By providing your Customer Reference Number in your scholarship application you are agreeing to the following:

Customer Confirmation

I authorise Centrelink to confirm with Monash University the current status of my Commonwealth Benefit and other details as they pertain to my concessional entitlement.  This involves electronically matching details I have provided with Centrelink to confirm whether or not I am currently receiving a Centrelink benefit.

I understand that this consent is for the period I am a student at Monash University.  I also understand that this consent, which is ongoing, can be revoked any time by giving notice to the Coursework Scholarships Unit at Monash University.

I understand that if I withdraw my consent I may not be eligible for the scholarships or bursaries provided by Monash University.

Income Confirmation

I authorise Centrelink to electronically provide a statement of information to Monash to assist in the assessment of my entitlement to scholarships and bursaries offered at Monash University.  I understand that the information provided by Centrelink may include where relevant, current or historical details of payments received, dependants, Centrelink deductions, income, assets and confirmation of my current address.

I understand that this authority, once signed, is effective only for the period I am a student of Monash University.  I understand that this authority, which is ongoing, can be revoked at any time by giving notice to the Coursework Scholarships Unit at Monash University.

I understand that I will be able to obtain a written copy of the statements at any time from either Monash University or Centrelink.
